Release:
In consideration of being allowed to participate in any program hosted by the Peabody Recreation, Parks & Forestry Department, I hereby expressly assume all risks of personal injury, death, or property loss arising in any way from my participation. I represent that I am physically fit and able to participate in this event/program. I release and agree not to sue the City of Peabody, its departments, officers, directors, volunteers, employees, sponsors, and agents from any and all liability and claims arising out of my participation, including those related to photographs, video, or other recordings.
If the participant is a minor, I, as the parent or legal guardian, hereby consent to their participation in all activities and field trips related to the Peabody Recreation, Parks & Forestry Department. I authorize the program directors and/or instructors to act as agents for the undersigned in consenting to medical, surgical, or dental examination and treatment, if necessary. Additionally, I release the City of Peabody, its facilities, and employees from any claims related to personal injury resulting from participation in the activities.
I understand and agree that photographs or recordings taken during the program may be used for promotional purposes. By signing below, I affirm that I have read and fully understand this agreement and accept its terms on behalf of myself and the participant.
